感染性心内膜炎瓣膜损害的外科治疗分析

Objective: To summarize the surgical experience of infective endocarditis (IE) with valve damage. Methods : From Jan. 2012 to Jun. 2014, 18 IE patients underwent heart operation in our hospital. There were 10 male and 8 female cases, with an average age of 37.2 years old. All patients received blood culture and 7 cases were positive (38.9%) before operation. Ultrasonic cardiogram (UCG) revealed various valve damage in all patients. 6 cases (33.3%) underwent emergent cardiac surgery. Removal of infective lesions and valve repair or replacement were completed during the operation. Results: 1 case with preoperative heart failure was died of severe infection and multiple organic failure at 6 d after operation. 17 patients were recovered. 2 patients with lasting post-surgical high fever were cured afer 2-week antibiotic treatment. During 4 to 30 months after operation, 15 cases (83.3%) were followed up and had no recurrent endocarditis and paravalvular leakage. Conclusion: Early diagnosis, enough and effective antibiotic therapy, optimal and early surgical treatment, removal of infective sites thoroughly and valvular function rebuilding could provide good therapeutic effect of IE with valve damage.
