Objective To present the case of a rare ocular side effect induced by topiramate use. Method This is a case of a 43‐year‐old transgender man who presented with unilateral ptosis of the right eye. Ptosis was noted after 4 years of topiramate therapy, prescribed as prophylaxis against migraines. Results While some adverse vision and eye effects associated with the use of topiramate have been documented, eyelid ptosis has only been reported in 0.14% of patients using the drug. Thus, topiramate‐induced ptosis is a very unusual finding. Conclusion Experience with this case has demonstrated that immediate discontinuation of the drug can restore eyelid function within 1 week.
