Certain testicular tumours may present with gynaecomastia 1–3 . Rarely in these cases is the testicular tumour not palpable at the initial clinical examination or at follow-up examinations. To enable early detection of these occult testicular tumours a serum hormonal profile followed by high resolution ultrasound examination of the testes are necessary 3–7 . We report a case of a Leydig cell tumour in which gynaecomastia antedated the clinical presence of a testicular swelling by 7 months.
