The first requirement for success in an operation for pterygium is to obtain a smooth surface, free of any residual portions at the place of the removed apex. There are various types of sharp instruments available for this purpose which, however, they do not always fulfil. By the use of the author’s special cornea knife, this has been found easy to achieve. The accompanying figures show the knife and its separate parts. A = head, firmly attached to bolt (D); B –handle, drilled and tapped to retain the bolt (D); C = movable jaw, drilled, has no internal thread; D = threaded bolt (blade guide); ÉL = blade (made from the blade of a safety razor). Before use the bolt is put through the openings in the blade and movable jaw. Then the handle, pushing on the movable jaw, is screwed over the bolt; when parts A, C, and ÉL have been pressed together, the knife is ready for use in which it is held like a pen and manipulated with the fingers. Bagóczky, A New Instrument for Peeling off Residual Portions ... 63 It can be used in all cases where scraping of the cornea is indicated. Advantages: Handy; changeable blade; does not force aside the corneal tissue, but helps to cut smooth surfaces. Maker of the knife: G. Otte, Budapest, Bástya-u. 12.
