Focal points The need for enhanced communication across the health care interface to improve unintentional discrepancies in prescribing has already been recognised in Government papers We aim to develop a standardised yet effective method of documenting drug‐related information across the health care interface The use of an evidence‐based approach to developing the study design ensures integration into current clinical practice Readmission rates within this population were found to be high, 20 per cent being readmitted within 60 days; therefore standardised yet effective provision of pharmaceutical care across the health care interface is essential By providing GPs and community pharmacists with written drug‐related information there is currently a trend towards delaying readmission to hospital
