Free floating thrombus exiting the left ventricle

A 75-year-old man with a history of previous myocardial infarction underwent two-dimensional transthoracic echocardiography as part of a routine follow-up evaluation. During the examination, a free-floating thrombus was identified in the left ventricular (LV) cavity. After a number of cardiac cycles, the thrombus suddenly flowed free out of the LV outflow tract into the aorta.
