Left ventricular free wall rupture after coronary artery bypass grafting

A 68-year-old man presented with new-onset angina 15 years following coronary artery bypass grafting. Cardiac catheterization revealed slow flow through a saphenous vein graft to a diagonal branch. The patient temporarily had cardiogenic shock. A 64-slice computed tomographic angiography showed basal–lateral rupture of the left ventricular free wall (Figs 1 and 2, Supplementary Videos 1 and 2).
