This editorial discusses a study on the issue of clean water access and the effect on infants exposed to HIV particularly in sub-Saharan Africa. It concludes that further investigation of extended maternal antiretroviral therapy is needed to address the longer duration of breast-feeding required in pathogen-rich environments and that although safe water interventions are likely to be helpful they are not a replacement for breast-feeding.
