Dry eye syndrome (DES) after blepharoplasty has many causes, but most relate to reduced eyelid function. The author provides guidelines for reducing the incidence of DES, explains causes, patient selection, procedures that may exacerbate symptoms, techniques to decrease risks, and provides "pearls" for reducing lid malposition and DES after blepharoplasty.
