Key Points Question What are the population-level trends in health care utilization and expenditures in the 2 years before death among adults in Ontario, Canada? Findings This cohort study found that health care expenditures in the last 2 years of life increased in Ontario from CAD$5.12 billion in 2005 to CAD$7.84 billion in 2015, and the intensity of health care utilization and deaths in hospital varied by resource utilization gradients. Meaning In this study, the observed trends demonstrated that costs and hospital-centered care before death are high in Ontario.
