In this paper I am going to look rather generally at some of the problems facing psychotherapists who like to use the transference as a basis for their interpretive work, but are faced with the difficulties that exist within the National Health Service, where the number of hours that one can devote to a patient and the heavy loading of substantially disturbed cases inevitably limit what one can hope to achieve in the way of dynamic change or long‐term exploration of the patient's psychopathology.
