Due to advancements in treatment of keratoconus the number of keratoplasty procedures has significantly decreased. This development leads to an increase in the number of patients with a therapeutically stable keratoconus developing a cataract. The selection of the best IOL is still a challenge in these cases. A new option for keratoconus patients with cataract might be the IC‐8 IOL (AcuFocus), a small aperture aspheric monofocal lens that follows the same principle as the KAMRA coneal inlay (AcuFocus).
