We present the case of an ectopic mandibular third molar at the lower border of the ramus of the mandible associated with a dentigerous cyst. An extraoral surgical approach using a submandibular incision was used to extract the tooth. We discuss other reports of a similar nature and detail our management of this condition and the rationale for an extraoral surgical approach.
