covers late onset dementia and MCI in elderly population, but little is known about Early Onset AD (EOAD) and early onset MCI (eoMCI), with age of onset<65 years. We aimed to establish differences in smell identification between EOAD, eoMCI and age-matched non-demented controls (NDC). Methods: 19 EOAD subjects with mild to moderate dementia, 17 with eoMCI, and 21 age and sex matched NDCwere assessed with the mini mental state examination (MMSE), Cambridge Cognitive Examination, Neuropsychiatric Inventory, Bristol Activities for Daily Living, and the University of Pennsylvania smell identification test (UPSIT). Results:EOAD participants performed worse on the UPSIT compare to eoMCI and NDC’s (p<0.001) and onMMSE (p1⁄4 0.003). People with eoMCI performed worse on MMSE compared to NDC (P1⁄40.01), however there was no significant difference between them for UPSIT scores. UPSIT score correlated with poor executive functions (r1⁄40.5, p1⁄40.028). Conclusions:Smell identification function has potential as a clinical measure to differentiate EOAD from eoMCI and age matched NDC. We found no difference in olfactory identification between eoMCI and NDC. Studies of older MCI patients showing significantly lower UPSIT scores relative to NDC may be attributed to a different disease process. Independent replication is needed to validate these findings.
