Key Clinical Message The urinary free cortisol ( UFC ) test is widely used for the screening of Cushing's syndrome. This case study illustrates the potential failure of the UFC test to correctly diagnose Cushing's disease ( CD ), indicating that the use of other complementary tests may be necessary to diagnose this disease in some cases.
