Key Clinical Message Several elective surgical managements have been reported regarding giant inguinal hernias ( GIH ), but no case of death. We here present a fatal case of bilateral GIH s in an extremely obese man with dyspnea. Bilateral GIH s are difficult to recognize in extreme obesity. Moreover, operation is recommended when GIH is diagnosed.
