現在,頚椎後方除圧術に椎弓形成術が用いられる頻度 は多い.一口に椎弓形成術といっても多くの術式が存在 する.実際の臨床の場では,各施設における椎弓形成術 に対する習慣や上級医師の好みなどにより術式は限定さ れ,下級医師が学ぶことができる椎弓形成術には偏りが 生じている.自主的に過去に遡って調べないかぎり,さ まざまな椎弓形成術を系統立てて学ぶことは困難である. 今回,世界に先駆け本邦の脊椎外科医が長きにわたる 苦労とともに,良好な成績を確立してきたさまざまな椎 弓形成術とその変遷を中心に,イラストを用いて述べた い.なお,ここで述べる椎弓形成術は hinge(若木折れ させるための溝)を作成し,椎弓を挙上することで脊柱 管の拡大を獲得する術式と限定し,一部の椎弓を切除す る partial laminectomyやいったん椎弓を外し,その後形 成的に再構築を行ういわゆる laminotomyは含まないこ ととする.
