頭頚部悪性腫瘍に対する放射線治療の進歩により患者 の予後は改善したが,晩発的な合併症として起こる血管 狭窄は,患者の予後や生活の質を下げる原因となり,そ の治療や予防は新たな問題となっている. 放射線治療後の頚動脈狭窄症(RI-CS)において, carotid endarterectomy(CEA)は術後の創部感染,脳神 経障害などが起こる確率が高いために high risk とさ れ,carotid artery stenting(CAS)のよい適応とされて きた.ただ,最近の報告では CAS を施行した症例で, CEA に比べ長期の経過観察期間中の塞栓性合併症や再 狭窄の率が高いとされている. 今回われわれは,症候性右頚動脈狭窄症に対し CAS を 施行した後にステント内血栓,同側脳梗塞を繰り返し, さらに対側の左内頚動脈狭窄が症候化したために CEA を施行した 1 例を経験した.RI-CS に対する治療選択の 難しさを示す症例であり,文献的考察を加え報告する.
