89 日サ会誌 2018, 38(1) はじめに TINU症 候 群(tubulointerstitial nephritis and uveitis syndrome)は原因不明の間質性腎炎にぶどう膜炎を伴う 症候群である.サルコイドーシスやベーチェット病など, 他の間質性腎炎とぶどう膜炎を合併する疾患との鑑別が 重要であり,臨床上しばしば鑑別が困難なことが報告され ている1,2).家族発症はTINU症候群ではかなり稀である が,サルコイドーシスは患者全体の1.8%と報告され,同胞 間での発症が多いとされている3,4).また,HLA-DR52など の特定のHLA型が遺伝的背景として報告されている4). 今回,弟がサルコイドーシスと診断されていたため,家 族性サルコイドーシスとの鑑別が重要であったTINU症候 群の1例を経験した.本症例は鑑別のために気管支鏡検査 を施行したが,TINU症候群における気管支鏡検査のまと まった報告は我々の知る限りなく,その検査結果からサル コイドーシスとの鑑別に難渋した.非常に示唆に富む症例 と考えられたため若干の文献的考察を加えて報告する.
