腔炎に対する内視鏡下副鼻腔手術後のプロピオン酸ベクロメタゾン(BDP)高用量鼻腔内投与療法の効果を 検討した。治療効果は副鼻腔粘膜の内視鏡所見と CT所見をスコア化して検討した。また,採取した粘膜に対 してグルココルチコイドレセプター(GR)のアイソフォームである GRα と GRβ の定性的観察を行った。 結果:症例全体(20症例,39側)では,術後の粘膜病変の内視鏡所見は,喘息合併群では非合併群に比較し て,有意に不良であった。続いて全副鼻腔の粘膜病変の CT所見は,症例全体では CTスコアの平均は9.4か ら3.2と有意な改善を認め,術後の CTスコアの合計は喘息合併群において平均値が3.9,非合併群では2.1で, 有意に喘息合併群が不良であった。また,粘膜の免疫染色では,篩骨洞粘膜の炎症細胞に GRβ の発現が強い 傾向が見られた。 まとめ:以上の結果より,好酸球性副鼻腔炎の内視鏡下副鼻腔手術後の BDP高用量鼻腔内投与療法は,有 用な治療法であることがわかった。気管支喘息合併群は非合併群に比べ有意に治療効果が不良であり,なんら かの追加治療が必要であると考えられた。 原 著
