Cet article tente de décrire les conditions d'exercice réelles d'un psychiatre itinérant. Puis à travers un bref retour historique, nous essayons de voir à quels courants s'alimente cette modalité de pratique : essentiellement ce qui s'est fait en France et aux États-Unis. Je reviens finalement sur mon expérience personnelle pour tenter de cerner les motivations plus profondes qui ont sous-tendu mon action, et explorer davantage le rôle que nous sommes appelés à jouer dans le contexte de «l'itinérance» ; généralement, il est plus polyvalent et requiert beaucoup de modération. L'article se termine sur les intérêts particuliers de cette modalité de pratique et quelques recommandations visant à favoriser d'une part l'insertion de psychiatres permanents dans les régions périphériques et d'autre part une meilleure intégration du travail des psychiatres permanents et itinérants.This article deals with the personal experience of the author as a travelling psychiatrist from Montreal into the north west territory of Province of Quebec. The first part presents a day's work under these circumstances.Than follows a brief historical review of what has been accomplish in Quebec and more particularly in the north west area concerning the development of psychiatric services. The travelling type of practice is then related to some trends and theories in community psychiatry. Next, I have tried to explore the internal motivation of the psychiatrist as well as of those who expect his visit. Finally, drawing from this experience as well as from other similar experiences, I try to set down some guidelines that should be helpful in organizing psychiatry on a more permanent basis and in integrating better the efforts of travelling and permanent psychiatrists in remote areas
