噴門側胃切除術 (以下, 噴切と略記) における有用な再建法につき検討を行うため, 胃切除範囲と食道残胃間の間置空腸に着目して再建術式別の術後QOLと機能の評価を行った. 術後1年を経過した2/3噴切空腸間置法再建5例 (2/3噴切群), 4/5噴切空腸間置法再建7例 (4/5噴切群), 4/5噴切空腸嚢間置法再建7例 (pouch群) を対象とし, 胃全摘空腸間置法再建12例 (全摘群) を参考対照として検討を行った. 2/3噴切群・pouch群の食事摂取は良好であり, 2/3噴切群では体重減少が少なく, pouch群では内視鏡による残胃の観察が全例可能であったが食道炎も見られた. また, 機能的には両群とも術前に近似した. 一方, 4/5噴切群と全摘群のQOLは不良で機能面でも大差を認めなかった. 噴門側胃切除術では残胃を可及的に温存すべきであり, 残胃が小さければ空腸嚢の作製により貯留能を増す努力が必要である.
