1986年 までの41年 間 に千 葉大 第2外 科 を訪 れ た食道 ア カ ラシ ア444例(男200例,女244例)の 病 態 を 食道X線,内 圧,筋 電図,内 視 鏡 お よび 胃液 検査 で検 討 した.本 症 は内圧 細 分類 に よるA1,A2,A3 型 の ご とき初 期像(食 道 緊張 型)とB型 の ご と き末期 像(弛 緩型)が あ る.本 症 に食 道癌 を合併 した 18例4.1%の うち病悩期 間20年 以上 の症例 は12例 であ った. 外 科治 療 を行 った275例 の遠 隔追求245例 中,良 好 は165例,軽 快 は43例 と併せ て84.9%が 改 善 された. 104例 に施 行 され た 胃弁 移植 術 の遠隔 追 求97例 中,良 好91例,軽 快6例 とすべ て 改善 した.教 室 で再手 術 を した30例(教 室13例,他 施 設17例)45件 の86.7%が 通 過障 害を理 由 とした.再 手術 の方 法 は 胃弁 移植 術14例 がいず れ も好 成績 であ った.
