頭蓋内椎骨動脈に窓形成を伴う頻度は 1~2%程度で, 臨床的にしばしば経験されるが 20),その窓形成部に脳 動脈瘤を伴う報告は稀である. 椎骨動脈窓形成の発生学的由来別にみた動脈瘤の報告 は,1胎生期にprimitive cervical segmental arteriesが 連結してできるprimitive vertebral arteryにおいて, segmental arteryが再灌流して生じた窓形成部に 1例 8) (Fig. 1A),2脳底動脈の原器であるventral longitudinal arteryとprimitive vertebral arteryを結ぶprimitive basilovertebral anastomosisが遺残して生じた窓形成部に 1例 1) (Fig. 1B),3頭蓋外椎骨動脈から起始した lateral spinal arteryが後下小脳動脈と吻合することで形成する椎骨動 脈のduplicationによる窓形成部に 6例 3,7,9,13,14),である (Fig. 1C).これらは全例が破裂例であることも特徴で ある. このたび我々は,未破裂で発見された頭蓋内椎骨動脈 症例報告 JNET 7:275-280, 2013
