頭頸部腫瘍に対する放射線治療後にlarge vesselの高 度狭窄(70%以上)が起こる率はChengら4)によると 11.7から16%,Elerdingら7)によると6.3%と報告され, 経過観察中に最も注意が必要な合併症の 1つと考えら れている.また,これらの血管狭窄により症状が出現す る率は14.6%と報告されている4).さらに,Chengら4)は, 放射線治療後に血管狭窄を起こす危険因子として,年齢 (60歳以上),喫煙,虚血性心疾患の有無を報告している. 血管狭窄は放射線治療後,数年から十数年と長期間経て 出現してくるが,症候性例では基本的に進行性であり時 期をみて外科的治療が必要となる場合が多いと考えられ る.そのため,経過観察中放射線照射部の血管病変の検 査を行うことは必須と考えられている. 今回,無症状で狭窄病変が発見され,その後経過観察 中に一過性脳虚血発作(transient ischemic attack,以下 TIA)が頻発し,外科的治療を行った症例を経験した. 頸動脈ステント留置術(Carotid artery stenting,以下 CAS)と浅側頭動脈―中大脳動脈(superficial temporal artery-middle cerebral artery,以下STA-MCA)バイパ ス術を組み合わせることにより良好な頭蓋内血流を得る ことが出来,術後経過良好であったので報告する.
