症例1:40歳 男性,右 頬粘膜癌平 1)腫 瘍の治療歴:2年 前 よ り自覚 していた頬粘膜の 栂指頭大の腫瘤 を主訴 に1973年12月,総 合病院を受診. 腫瘤部 よ りの試験切除に て扁平上皮癌 の組織診が下 され たため,同 院にてコパル ト60,推 定総量8,000radsの 照 射を受け,腫 瘍 は消失 し,そ の後の再発は認め られなか った. 2)骨 障 害の治療歴:1975年12月 頃 よ り開 口障害が発 現,1976年4月 断 当科を紹介 され来院 した.初 診時, 右顔面部 に約4cm大 のほぼ円形の皮膚欠損 と,そ の周 囲皮膚 の瘢痕,色 素沈着,発 赤 があ り,皮 膚欠損部は凹 凸不 正で偽膜に被覆 され,易 出血性 で一部下顎枝の骨露 出が認め られた(写 真1).パ ノラマX線 所見 では,右 下 顎枝部 の骨 が全体 にやや粗 な感 じであったが著変は なか った(写 真2).放 射線性皮膚炎な らびに 下顎骨骨 髄 炎の臨床診断 のもとに抗生物質,消 炎酵素剤の投与と 局所 清掃 に より消炎 をはか りつつ,腫 瘍 の 再 発 を懸 念 し,適 宜,細 胞診,組 織診を行い慎重な経過観察を行っ
