患 者:■61歳 男性 ■. 主 訴:顎 下腺,耳 下腺,舌 下腺,涙 腺部 の対称性腫 脹および 口腔 内乾燥. 家族歴,既 往歴:特 記すべ き事項はない. 現病歴:昭 和48年5月,右 側下顎臼歯を抜去,翌 日よ り同歯 肉部,両 側顎下部が無痛的に腫脹,さ らに数 日後 には拇指頭大 までに腫大 した.以 後,内 科,耳 鼻科,外 科さらには某大学病院 を受診 し,諸 検査,薬 剤投与を う けたが,顎 下腺 の症状 にはなん ら変化 はな く,そ のまま 放置 していた ところ,49年1月 ごろ よ り左右耳下腺が次 第に,無 痛性 に,対 称性に腫脹 しは じめた.さ らには4 月 ごろ より,左 右上眼瞼周囲に不快 感をおぼ え,視 力低 下 を 自覚す るよ うにな って きた.ま た,唾 液分泌 も抑制 され,口 腔 内乾燥感が著明 とな り,嚥 下困難や 口角炎を 認め るよ うにな って きた.再 び,某 外科を受診 し,当 大 学病院を紹介 され,49年5月 ■ 日に来院 した.
