the current study presents the surgical digitally guided planning for the MARPE technique which consists of a MARPE three-dimensional positioning  and its mini-implants. This technique includes the simulation of the perforations areas on the midpalatal and transpalatal sutures.Methods: the MARPE positioning without colliding with the lateral tissue of the palate, and a bicortical positioning of each mini-implant are performed by superimposing the patient's files (STL and DICOM). The MARPE EX device allows individualizing the height of the mini-implants rings. The location of the palatal sutures is performed to plan the insertion of the perforation drill, avoiding collision with structures such as the nasopalatal duct.  Results: a guide that reproduces MARPE positioning and another guide that reproduces the perforations are produced, providing reproducibility as performed virtually.Conclusion: MARPE guide allows the virtual planning of MARPE respecting the anatomy of each patient, even in severe cases of atresia and asymmetry.
