Objective:We examined the duration of specific stages in the end-of-life period of older adults living at home, and the changes in the frequency and other structures of nurses’ home visits during these stages. We compared cancer and non-cancer cases. Method:Thirty-one nurses from home care nursing stations where “good” nursing care is provided, were interviewed concerning the practice of end-of-life care for patients that were 65 years and over, in both cancer and non-cancer cases. The end-of-life period was classified into onset, maintenance, worsening, and dying stages, and the differences in duration, visit frequency and other structural characteristics in each stage were statistically compared between cancer and non-cancer cases. Results:The length of the onset and worsening stages among non-cancer cases was significantly longer and more varied than that among cancer cases. The visit frequency for cancer cases became significantly higher in the dying stage, while it became higher from the worsening stage among non-cancer cases. On-call home visits were provided for almost half of the non-cancer cases in the dying stage. Conclusion:The findings suggest that the necessary end-of-life care, and the frequency of nurses’ home visits were different between cancer and non-cancer cases. Sci., Vol. 31, No. 1, 2011 69 I.緒   言 死が迫った際の療養希望場所として 6割の日本人 が自宅で過ごすことを望むが,それが不可能だと思う 人も 6割で,その理由としては家族の負担と急変時の 不安が上位を占める(終末期医療に関する調査検討会, 2005).死亡場所は病院が 80.0%で自宅は 12.2%に とどまり(厚生労働省,2006),今後,高齢者が望む 場所で死を迎えることをこれまで以上に可能にするた めには,療養者および家族への地域支援体制の整備が 望まれる(Bee et al.,2009).訪問看護は在宅で死 を迎える上で重要な社会資源であるため,在宅高齢者 の終末期の過程を解明し,終末期に提供されるべき訪 問看護をモデル化することが急務である. 成人のがんの終末期の時期についての報告はいく つかある.川越(1996)は,がんによる終末の経過 を「在宅終末期ケア準備期」「開始期」「安定期 」「終 末期」「臨死期」に分け,各期間を「在宅ケア開始前」 「開始後 1週間まで」「開始期後より終末期に入るまで の期間」「死の約 1週間前」「死の前数日から 24時間 以内」と定義した.恒藤(1997),柏木(2000)は「終 末期前期」「中期」「後期」「永眠直前期」と分け,各 期間を生命予後が「6ヵ月から数ヵ月」「数週間」「数 日」「数時間」と示した.これらは成人を中心とした がんの経験に基づくものである.がんとがん以外で は,死亡前 1年間の身体機能低下のパターンとそれ に伴うケアの違いが指摘されているが(Teno et al., 2001;Lunney et al.,2003),がん以外の疾患の終 末期の経過については報告が限られている(平原ら, 2006).さらに,在宅高齢者の終末期において,訪問 看護師がどの程度の訪問頻度などで支援を行うべきか に関する実証的な研究はない.以上のように,特にが ん以外の疾患をもつ高齢者の終末期の経過を解明する こと,さらにその間の訪問看護の提供体制等の特徴に ついて実証的に明らかにすることが,今後,高齢者の 終末期を支援する訪問看護体制を確立する上で重要と 思われる. そこで,本研究では,「良い」終末期ケアを行って いると判断される訪問看護ステーション(以下ステー ション)における訪問看護の実施経過についてのデー タをもとに,終末期の時期別の期間と各時期に実施さ れた訪問看護の訪問頻度および体制を明らかにするこ とを目的として検討を行った.特に,がんとがん以外 の疾患における経過と訪問看護提供の実態を比較した. なお,本研究は訪問看護における高齢者への望ましい 終末期ケアを構造化することを目的とした調査の一部 であり,その目的で収集されたデータをもとに分析し た. ここでは「終末期」を,治療を尽くしても疾患の治 癒が期待できず近い将来死が訪れる状態と定義した.
