Intermittent hydrarthrosis is an uncommon situation, usually seen in major joints, most frequently in knee, with episodic synovitis attacks. It can be treated with low dose colchicine, non-steroid anti-inflammatory drugs, gold salts, intra-articular steroids and synovectomy. In this article, we presented a 32-year-old patient with intermittent hydrarthrosis and carrier of familial Mediterranean fever resistant to arthroscopic synovectomy, radioactive synovectomy, repeated joint injections, colchicine, and non-steroid anti-inflammatory drugs. His symptoms of intermittent hydrathtrosis could be controlled by step-up combined disease modifying antirheumatic drug treatment hydroxychloroquine plus methotrexate in addition to colchicine.
