Objective: to perform a retrospective clinical assessment of direct composite resin restorations (class V) held at the School of Dentistry of the Federal University of Rio Grande do Sul. Materials and method: the study included adult patients of both genders, who were treated with class V restorations in the anterior area of either upper or lower dental arch including premolars, performed with the following composite resin types: microfilled, microhybrid, nanofilled, and nanohybrid, as well as restorations that were in service for at least 6 months. Smoker patients with either occlusal problems or severe parafunctional habits, poor oral hygiene or with special needs were not included in the study. Two evaluation methods were used by a trained and blind examiner: the modified USPHS and the modified FDI. Data collected by the clinical assessment were tabulated for descriptive analysis, according to both methods applied. Results: forty seven cervical restorations were assessed with a time interval in service from 6 months to 18 years (mean 6.5 years). Twelve restorations (26%) were found and considered inadequate by the methods applied. The major causes of failure were: recurrence of cavities, erosion, abrasion, or abfraction (12%) by the FDI method, and fracture and retention loss (11%) by the USPHS method.
