This presentation presents findings and lessons from a study of the use of digital health technologies (DHT) to support integrated care in four EU countries. It is gives practical examples of how DHT has been used by pionnering integrated care schemes for people suffering from serious mental health challenges (Sweden); diabetes; (The Netherlands); and older people with significant health conditions (Germany) as well as a hospital at home service in Spain. The session considers how DHT helps integrate patients into the care team and supports self-care and co-care, as well as implementation issues and privacy and security issues. Lessons for others and future developments in the emerging digital revolution in health and social care are discussed.

Improving quality with digital health technology supporting coordination and self-care: findings from 4 EU countries in the EU integrate project
