4人の遷延性意識障害者のエネルギー消費量, 窒素出納を実測した。エネルギー消費量は, 早朝と栄養剤投与後2時間で間接熱量測定法により測定し, 前者を基礎代謝, 後者を安静時代謝とした。結果は, 同年齢, 同体位の健康成人の基準値と比較した。その結果, 基礎代謝量は4人の対象者とも顕著な低下を示し, 低下率は2748%にわたった。甲状腺ホルモン濃度の低下はみられなかった。体内筋肉量を反映するクレアチニン係数, 筋肉代謝速度の指標としての3-Met/Crn比は減少していた。体脂肪率は3人で高値を示した。これらより, 遷延性意識障害者の基礎代謝の低下は, 長期間の安静状態による活性組織量の減少が一因と考えた。安静時代謝量も低下したが, 基礎代謝量の1.1倍程度の値であり, 遷延性意識障害者においても食事投与後のエネルギー消費の亢進の存在を示唆した。窒素出納値は3人で負の出納値を示し, 2人についてはたん白質量を変化させても窒素出納には変化はみられなかった。
