68歳男性。数年前より外陰部の紅斑と掻痒に気付く。近医皮膚科より乳房外Paget疑いにて当科紹介。外陰部・右腋窩に境界不明な紅斑脱色素斑が多発するも, 左腋窩は異常なかった。生検で外陰部・左右腋窩の3ヵ所ともPaget細胞を認めた。左腋窩は肉眼的に異常なかったが, 組織学的にPaget細胞が存在したOccult Paget's diseaseだった。画像上, 転移・内蔵病変はなく, 血中CEA値も正常。RI法, 色素法による両側鼠径センチネルリンパ節生検は陰性。外陰部・右腋窩は2cmマージンで, 左腋窩はPaget細胞陽性部位より3cm離し, 筋膜上で切除・分層植皮を施行。外陰部は一部腫瘍断端陽性で追加切除を行った。術後9ヵ月経過する現在, 再発・転移なし。今回, occut病変のPaget細胞同定に, Cytokeratin 18染色が有用だった。
