78歳女性。初診の半年前より頭頂部に小指頭大の腫瘤が生じ, 初診時は1.5×2cmの赤色有茎性の腫瘤がみられた。5mmはなして帽状腱膜上で切除し植皮をしたが, 術後1年後まで再発はない。組織学的に腫瘍の大部分を占めるsarcoma様部分と腫瘍下部の一部に腺様構造部がみられた。両者の間には移行がみられ, 免疫染色もほぼ同じ傾向を示したので, 一元的に考えるのが妥当と思われた。以上より, 今回の腫瘍は, 先に存在するeccrine spiradenomaははっきりしないが, Ackermanの記載したsarcomatous typeのspiradenocarcinomaとしてよいのではないかと思われた。
