Good interpersonal communication improves and facilitates human relationships, granting effectiveness, but the practice of modern Medicine does not consider it as a priority. Critical Care Units are not an exception. Although care is taken efficiently against illness, the patients and their families are not always satisfied with communication between them and caregivers and, at the same time, this is usually identified as the most important and least accomplished factor in the quality of care, mainly in end-of-life processes. Furthermore, advances in life support allow patients with no expectative of recovery to keep on living. Consequently, some patients only die after a decision to forego life sustaining therapies. Decision making about these topics can be very difficult for clinicians. The consensus among different professionals is needed and the involvement of the patient’s family is crucial too. These decisions take place in a very high emotional climate, thus communication, respect and compassionate attitude are essential. The welfare pressure, the lack of training of clinicians in the managing of complex situations, fear, or even the perception of death as a failure of Medicine, can explain, among other reasons, the serious deficit in this field. Integrating the palliative medicine and the skills of communication among the competencies needed for critical medicine would allow to improve the attention to the critical patient who dies and also to help his/her family facing the difficult situation of accepting the reality that has been imposed to them.La buena comunicación interpersonal, que mejora y facilita las relaciones humanas, dotándolas de  eficacia y calidez, no suele ser una prioridad para la moderna Medicina Hospitalaria. Las Unidades de Cuidados Críticos, no son una excepción. Aunque en ellas se actúa eficaz y diligentemente contra la enfermedad, con frecuencia tanto pacientes como familiares se quejan de la falta de comunicación con los profesionales, a la vez que la identifican como uno de los factores principales en la calidad de la asistencia, sobre todo en los procesos al final de la vida. Además, los avances en el soporte vital, permiten que pacientes sin perspectivas de recuperación puedan sobrevivir largo tiempo por lo que es habitual que algunos enfermos sólo fallezcan después de decisiones de limitar tratamientos. Estas decisiones no son fáciles; precisan del consenso entre distintos profesionales, han de implicar a los familiares del enfermo y tienen lugar en un clima de alta tensión emocional por lo que una buena comunicación es imprescindible. La presión asistencial, la falta de formación del médico en el manejo de situaciones complejas, el nulo reconocimiento curricular de las actitudes, el miedo, o la percepción de la muerte como fracaso de la Medicina, pueden explicar, entre otras causas, el grave déficit en este campo. Integrar la Medicina Paliativa y las habilidades de comunicación entre las competencias de la Medicina Crítica no solo permitiría mejorar la atención al paciente crítico que fallece sino también ayudar a su familia en el difícil trance de aceptar la realidad que se les impone
