腹腔鏡下 手術の手技 お よび使用機械の開発 ・改 良はめざま しい進歩 をとげてお り、その適応は婦 人科 良性疾患 のほぼすべてが対 象 となって いる。 良性卵巣嚢腫 に対す る腹腔鏡下手術 は手術侵襲が 少な く、入 院期間の短縮 、早期社 会復帰 などが可 能であ り、 開腹術 よ り多 くの利点があ るといわれ ている。 しか し卵巣腫瘍 の良性 ・悪性 を術前 に完 全に鑑別す ることは不可能であ り、 また腹腔鏡下 手術 の場合 、ほ とんどの症例 において術 中被膜破 綻が陽性 となるため、悪性卵巣腫瘍の可能性 につ いては十 分に注意 しなければな らない。 当科 にお いては術 前に超 音波検査、MRI検 査、腫瘍マー カ ー などを川いて卵巣腫瘍 を評価 し、悪性腫瘍が除 外で きた場 合は、ほ とん どすべ ての症例 に対 して 腹腔鏡下手術 を行 なって いる。今回、術前評価 に て良性卵 巣嚢腫 と診 断 して腹腔鏡下手術 を行 った が、術中あるいは術後病理診断 にて悪性卵 巣腫瘍 と診断 された症例 を経験 したので報告 する。
