An effective cough is essential for maintaining respiratory health. There are many causes of cough, the majority of which originate in the lungs, but there are also nonpulmonary etiologies. Chronic cough in children 14 years and younger usually is defined as a daily cough lasting 4 or more weeks. Common causes of specific cough in children include suppurative lung disease and other chronic infections (e.g., protracted bacterial bronchitis, recurrent aspiration, bronchiectasis, mycoplasma, pertussis, and parapertussis) and cough-dominant asthma. Other important causes include tuberculosis and foreign body aspiration. Less common causes include congenitalanomalies, tracheoesophageal fistula, neoplasia, or psychogenic processes, and rarely other nonpulmonary processes. A detailed medical history, the age when cough started, age of the patient, time and character of the cough, factors that trigger coughing, previous illnesses, surgical procedures, previous treatments, family history, environmental conditions, and household must be questioned. Etiologies of chronic cough in children are different than in adults, and thus clinicians should use pediatric-specific cough guidelines.
