症例は, 拡張型心筋症・気管支喘息の既往があり, 呼吸困難を起こした後, 気管内挿管され低酸素血症・嚥下障害をきたした63歳の男性である.嚥下障害は軽度であったが, 咽頭食道透視検査で誤嚥を認めた.高次脳機能障害が疑われ, 神経心理学的検査を試みたところ, 記銘力や理解力の低下を認めた.嚥下障害に対しては, 理学療法を主とした間接的機能訓練を行った.機能訓練を行う理学療法士が食事に付き添い, 厳重な管理下で経口摂取を行った.その結果, 自分自身で下咽頭に貯留した食物残渣を喀出するなど自己管理ができるようになった.咽頭食道透視検査で誤嚥がなくなり, 3食経口摂取が可能となって退院にいたった.記銘力・理解力が低下している場合でも, 神経心理学的所見を含め, 臨床像から問題点を明らかにして機能訓練を進めることが必要であると考えた.
