骨切りにより上顎の前方移動を行うと, 術後開鼻声の増加や構音の変化を招来することがある.再手術や構音訓練の適切な時期を知ることを目的として上顎前方移動術を行った口蓋裂患者の術前術後の開鼻声と構音の経過をみた.対象は口蓋裂患者16例 (男9例, 女7例) , 手術年齢は16歳から26歳 (平均19.6) 歳であった.術後, 開鼻声が増大した症例が10例あったが, その1/3は術後1年以内に術前の状態に戻った.術後開鼻声の改善を示した症例は, 術前の開鼻声がないか, またはあってもごく軽度のものであった.鼻咽喉閉鎖不全に直接または間接に関連している子音の鼻音化, 咽頭摩擦音, 声門破裂音のような構音障害は術後, 悪化するか変化しなかった.一方, 口蓋化構音や/s/の口唇音化のように上顎と下顎の位置関係や口腔容積と関連する構音障害は改善した症例があった.口蓋化構音の改善は聴覚印象とパラトグラムによって確認された.構音の変化は術後1年以内におこった.
