要旨:当科では 2005 年 3 月より,一般歯科診療所と病診連携を開始し,積極的に口腔ケアを導入して きた。今回頭頸部癌再建手術症例を対象とし,口腔ケアと術後合併症について検討を行った。対象は 2004 年 6 月~ 2008 年 9 月に当院で同一術者により遊離再建手術を行った 95 例である。症例の内訳は 男性 76 例,女性 19 例であり,原発部位は口腔 33 例,中咽頭 25 例,下咽頭・頸部食道 23 例,その他 14 例であった。再建材料は腹直筋皮弁 39 例,前外側大腿皮弁 19 例,空腸 29 例,その他 8 例であった。 術後合併症発生率は,非口腔ケア群が 65.0%であるのに対し,口腔ケア群では 40.4%と有意に(p = 0.018)低下が認められた。これより口腔ケアの導入は術後合併症の防止に有用であると考えられた。 キーワード:口腔ケア,術後合併症,頭頸部癌,再建手術,歯科開業医との病診連携
