We report a rare case of the stress fracture of the distal tibia and fibula after total knee replacement. We know that stress fractures of the pubis, femoral neck, patella and proximal tibia are often seen after TKR, but stress fracture of the distal tibia is not known. Seven months after TKR, a 70 year-old-femal complained of rt. ankle joint pain. Three weeks after onset, the distal tibia and fibula were fractured and varus displaced. And tibial compornent of the total knee system was fixed in 7 degrees varus position.
