Pathogenesis of ossification of posterior longitudinal ligament of the cervical spine (OPLL) is not well understood. We have been studying its bone metabolism using bone morphometrical technique. We made undecalcified ground sections of OPLL, vertebral bodies, and ilium, which were surgical materials with Villanueva bone stain. Trabecular bone specific volume was slightly higher in OPLL than in CSM and in normal bone. Mineral appositional rate (MAR) might be higher in OPLL. The edge of OPLL, where the mechanical stress is influential, showed high turnover, whereas the center of OPLL showed low turnover.
