心房中隔欠損(atrial septal defect : ASD)の閉鎖術ではシャント遺残に気をつけなければならないが,欠 損孔の下縁がない場合には特に注意を要する.初回手術から 37年後に ASDの遺残短絡に対し再手術を行っ た症例を経験した.17歳時に他院で ASD閉鎖術を受けた 54歳女性で,労作時の息切れを主訴に前医を受 診し当院に紹介となった.精査の結果,左側相同および心房中隔欠損の遺残短絡とそれに伴う肺高血圧症と 診断し手術を行った.術中所見では肝静脈の開口部に跨がるように遺残 ASDを認めた.欠損孔の上縁から 頭側に向かって前回手術時に直接閉鎖を試みたと思われる縫合線を認めた.いわゆる“下縁欠損 ASD”に 対し下端の閉鎖が不完全であったための遺残短絡と考えられた.再閉鎖を行い,術後は心合併症なく自宅退 院した.ASD下縁の閉鎖は特に確実に行うよう心掛け,術後のチアノーゼ,心不全の発生に留意する必要 がある.日心外会誌 48巻 5号:313-315(2019) キーワード:成人先天性心疾患;心房中隔欠損症;左側相同;遺残短絡;再手術
