症例は 89歳女性.労作時息切れおよび心拡大に対し内科的治療を行われるも症状の改善認めず.精査目的 にて施行された胸腹部 CTにて心臓を圧排する巨大な心膜腫瘍を認めたため手術目的にて紹介となった.術 前腫瘍の鑑別はできなかったが,心臓の圧排による心不全所見を認めたため診断を兼ねて腫瘍摘出の方針と した.腫瘍は,左室前壁の心外膜に起源し左前下行枝(LAD)に沿って約 3×10 cmの茎を有した.LAD を完全に巻き込んでいたため,人工心肺下,心停止下に腫瘍摘出術を施行した.LADも一部合併切除を要 した.術後経過は良好で術後 37日目に独歩退院となった.病理組織診断は,原発性孤立性線維腫瘍(solitary fibrous tumor ; SFT)であった.心膜発生 SFTは稀であり,なかでも左室起源の SFTはきわめて稀 な疾患である.若干の文献的考察を加えて報告する.日心外会誌 45巻 4号:192-195(2016) キーワード:孤立性線維腫瘍;SFT;原発性心膜腫瘍;心臓腫瘍
