大動脈二尖弁(BAV)は,遺伝性の結合織異常であり,弁機能異常がなくても,大動脈基部や上行大動脈が 拡大するおそれがある.今回 BAVの形態と上行大動脈拡大の関連,および上行大動脈拡大の形態や上行大 動脈壁の病理学的な特徴について検討した.2004年以降に大動脈弁膜症に対して大動脈弁置換術(CABG 併施を含む)を施行した 276 例中,BAV と診断した 60 例(21.5%)を対象とした.BAV の形態は, Sievers分類を用い,上行大動脈拡大の形態は,造影 CT(3D構築)にて評価した.病理学的な特徴は,嚢 状中膜壊死の程度を aortic wall scoreを用いて評価した.BAVで最も頻度が高い Type 1のなかで,L/R 型に上行大動脈拡大が多く(48%),BAVの形態と上行大動脈拡大の関係が示唆された.上行大動脈拡大の 形態的な特徴は,asymmetric dilatationが多かったことであるが,基部拡大例ではすべて symmetric dilatationを呈していた.病理学的な特徴は,上行大動脈が拡大していない症例も含めて全例に嚢状中膜壊死を 認めた点であった.Aortic wall scoreは基部拡大群で有意に高値であり,上行拡大群よりも壁の変性が高度 であるという結果であった.結論として,BAVの場合では基部拡大が軽度でも,積極的な基部置換術が必 要であると考えられた.日心外会誌 42巻 2号:89-93(2013) キーワード:大動脈二尖弁,上行大動脈拡大,aortic wall score
