左房瘤は稀な疾患である.今回,僧帽弁輪変形をきたし僧帽弁閉鎖不全症を合併した左房瘤の 1手術例を経 験したので報告する.症例は 71歳男性.呼吸困難,下腿浮腫を主訴に近医を受診した.精査の結果,左房 後壁の冠動脈回旋枝と冠静脈の間から心尖側へ突出する左房瘤と僧帽弁閉鎖不全症,および重症心不全と診 断された.術中所見では僧帽弁輪(P2〜3)に接する 5×6cm大の左房瘤を認めた.瘤の一部が弁輪に達し ており,弁輪が心尖方向へ落ち込み,変形していた.弁尖逸脱や腱索断裂は認めず,左房瘤による弁輪変形 に起因した僧帽弁閉鎖不全症と判断した.瘤を左房内腔より僧帽弁後尖の弁輪と平行方向に背部に位置する 回旋枝と冠静脈に注意しながら縫縮閉鎖し,弁輪形成術を施行した.重症心不全に対して心臓再同期療法を 併施した.術後心臓 CTで回旋枝と冠静脈に変形なく,瘤の縮小を確認した.心エコーでも僧帽弁逆流を認 めず経過良好で退院した.日心外会誌 42巻 4号:333-336(2013) キーワード:左房瘤,僧帽弁閉鎖不全症,僧帽弁形成術
