症例 は61歳, 女 性. 健診 で 心電 図異 常 を指摘 された. 精査 の結 果, 大 動脈 弁 閉鎖 不 全 (AR)を 伴 う右 冠, 無 冠洞 の Valsalva 洞 動 脈瘤 と診 断 され た. 瘤径 は右 冠 洞41×40mm, 無 冠 洞38×28mmで 左 冠 洞 は正 常 形 態で あ った. 大動 脈弁 尖 に異 常所 見 を認 め ず弁輪 径23mm, ST junction (STj) 径27mmで あ った. 右 冠 洞 の拡 大 に よ り弁輪 が 変 形 し右 冠尖 が 逸脱 す る こ とに よ りARが 発 生 して い た. Root remodeling を応 用 した 自 己弁温 存 術 式 を選択 した. 右 冠, 無冠 Valsalva 洞 動 脈瘤 を完全 に切 除 し. 左 冠 洞 は温 存 した. 三 つ の交 連 下 に 弁 輪 縫 縮 術 を行 い, 26mm Hemashield(R)人 工 血 管 の2/3周 を トリ ミ ン グ して Yacoub の remodeling 法 に準 じて Valsalva 洞 欠損 部 を補填 した. 右冠 動 脈 をボ タ ン状 に切 り抜 き Carrel patch を人 工 血管 に縫 着 した. 右冠 動 脈の spasm か ら同部 へのCABGの 追加 とIABPを 要 したが, 術 後経 過 は良好 で ARは 消 失 した. 日心外 会 誌31巻6号: 399-403 (2002)
