症例は37歳 女性. 完全房室ブロックにて左鎖骨下にPM植 え込み術の既往があった. 今回, 左鎖骨下 generator 交換後に創部感染を併発 したため, 左鎖骨下generator 除去, 右鎖骨下に新規PM植 え込み術が施 行された. 術後2カ 月目頃より発熱, 湿性咳が出現し始め, 当科に紹介入院 した. 心臓超音波検査で三尖弁 直上のPMリ ー ドに高輝度を有し, 浮遊する疵贅を認めた. 胸部CT検 査で両肺野末梢を中心に多発性結節 影を認め, 一部には空洞形成も認めた. 右心系IEか らSPEを 合併したと考え, 開心術を施行 した. 右房を 切開 し, 新旧PMリ ー ドと三尖弁に付着 した疵贅をすべて除去し, 心筋電極を右室横隔膜面に植え込んだ. 疵贅の細菌培養では methicillin sensitive Sfaphylococcus aureus (MSSA) が検出された, 術後長期の 抗生剤治療を要 したが, 肺病変も完全治癒した. 感染PMの 積極的除去と術後抗生剤投与で救命しえた1例 であった. 日心外会誌31巻2号: 124-127 (2002)
