SJM弁 を用いて三尖弁置換術を行った12例 の遠隔成績について検討 した. 対象は, 男性5例, 女性7例 で, 手術時年齢は6~62歳, 平均40±19歳, 15歳 以下の小児例は3例 で, 術後観察期間は1~16年, 平均 11年 であった. 早期死亡はなく, 3例 が遠隔期に死亡し, 死因は心不全・突然死・再僧帽弁置換術後の術後 呼吸不全であり, 累積生存率は5年100%, 10年80%, 15年60%で あった. 4例 が9~14年10カ 月, 平均 11年5カ 月で再弁置換術を要 した. いずれも血栓弁で, 再手術回避率は5年100%, 10年78%, 15年29% であり, 10年 以降急速に低下する傾向がみられた. SJM弁 による三尖弁置換術後は, 抗血小板剤を併用す るなど, 厳重な抗凝固療法が必要であると思われる. 日心外会誌30巻6号: 277-279 (2001)
